The library in Greenwich Connecticut had a small 16mm theater (Norelco) in the basement, in between the men's and ladies' bathrooms. My girlfriend and I would have to split up to get in. For a joke we would say to each other; "Journey's end with lovers meeting ..."

In NYC there was a theater (the Elgen that ran silents and other classics) on the west side that had a ladder to the booth. An operator was leaving, lost his grip, and almost fell on a bunch of us below.

In Connecticut, they had made a law against ladders (including "ship's ladders" and circular staircases) very early, so I don't think that was a problem here.
