I work for an independent operated by an individual from '90 till this past April, and since then by a non-profit. Shortly after he opened, the last guy was advised by his attorney to create a manual just to cover his ass on a variety of issues. It is pretty encompassing. For a long time every new hire had to read it and sign that they had. Gradually that requirement faded away. The book is now on a shelf in a closet in the office and hasn't been touched in years. The important things are still done the way prescribed in the manual and the rest ignored. The best part of not being part of a giant organization is being able to make a decision without consulting "The Book".
