1) A hard, tender spot on your forehead would likely not be classified as a knot, since it would probably be a tender spot of something other than muscle tissue.
2) A chiropractor (or any other healthcare practitioner) who saw someone complaining of seeing double and passing out would be extremely unlikely to focus on any muscular knot. They'd send the person out for xrays and MRI's and do exams to test the person's nervous system as well. Seriously, you don't know what you're talking about in any way. Heck, if you went to a chiro seeing double and passing out and the guy only rubbed on a sore spot on your forehead, I'd be telling you to call your local board to investigate him/her pronto.

A "knot" is a general term and can be used to cover many things.
Yeah it's a pretty vague term at best.

Any visit to a health care provider requiring a report or documentation of some kind will always have something on it justifying any treatment unless the visit is purely diagnostic.
And the documentation will be "pt complained of pain at the level of x/10 with tender palpable nodules at [these spots]."
