Pretty simple, doesn't matter what type of food you eat! Fact is, when you take in more calories than your body uses, they store up. When you expend more than you take in, your body turns to fat for fuel.

Depends on how underweight. A pound underweight and anorexic would be very different of course. I believe the healthy body fat for a man in his 20s or early 30s is approx. 9-15%.
Risks associated with being more than a few pounds underweight for a long period of time : bone loss, low testosterone levels, muscle loss or atrophy from body turning to muscles for energy, intollerance to cold, greater stress on organ function.
So... being too thin can be just as unhealthy as being overweight.
If you're concerned you're on either end of the spectrum and need to change, see your doctor or a doctor and a nutritionist/dietician.
