Mike, I could post plenty of photos of buildings with Cinema or Theatre(er) on their marquees. And plenty that don't
If you go really far back, like the teens, theatre didn't have marquees, and you'd be right about those. Many of them were retrofitted with marquees in the 30's and 40's and then got the extra words added.
I do, however, have pix of the Crown Theatre in Mobile, AL (1911) and The Strand Theatre in Montgomery (1915) with "Theatre" on the front of the building.
As for chains like Majestic and Orpheum, or the palaces of the 20's, they were also vaudeville houses, so certainly didn't want to call them cinemas. Ultimately, it probably just came down to whatever the person building it want to call it. I'm still fond of "Theatrorium."
