I think generally this is done by the architect, who specifies construction & items used, then the local contractor does the installation following the instructions supplied with the product (if you're lucky!)

Things like sound isolation in walls is generally architectural design like double, floating walls. The frames, doors & windows are generally units specialized for sound isolation (double panes, one tilted, etc.) I don't have the Holy Grainger Catalog here WHERE CAN IT BE? WHERE CAN IT BE? makes some isolated door & window units used in studios. Kawneer's specialty is aluminum products, so their doors & windows tend to that late 70's brushed aluminum & glass shopping mall monolithic look. They're sturdy, high-traffic commercial/industrial two pane doors which could probably be used as hellacious plate capacitors if you could charge them at a hydro plant or something.

Sound isolated wood doors, even big, heavy ones, seem to tend to get chewed up in heavy usage environments.
