I saw Wreck-It Ralph 2 today in 4DX. Although the actual movie was flawless in presentation, something odd happened during the adverts and trailers. The seats began to yaw and pitch violently; scent and water was going off when the screen was simply showing an advert for the theatre's unlimited programme. It seemed as if the 4DX system thought it was playing that trailer with the seat racing through the street (to show off the effects) but the wrong video was playing!

I have very little experience of how 4DX works, but you'd think the effects would be embedded in the video file like with D-Box. Or are there separate files with timecode that could be mixed up when ingesting?

Motion and effects files are stored into "slots" and then activated with macro from the server and syncing with the CPL playing. So if a slot was called with the wrong CPL that would produce what you experienced.
