Mag/Optical: 4-track Dolby A mag/mono optical
Dolby A SVA prints of this may exist, though all of the 35 prints I have ever seen were mag/opt.
70mm:
6 reels (see above)
Dolby A NR mag tracks
Unless new prints have been struck, expect this to be one pink movie: I saw a 70mm print of it in 1996 and it was well on its way to eternal pinkness...

Some mag prints would have been in Quintaphonic format running L-mag & R-mag through a quadraphonic matrix (similar to some home quad systems of the early 70's) to drive L & R front and L & R rear speakers. Center was directly from C-mag.Tommy was also the first Dolby SVA feature, AFAIK.

Some 35mm Dolby SR-encoded prints were struck, I believe, around 1994 for retrospective screenings. Don't know if the print(s) are still available for screenings.There *might* also be some SDDS print(s), as Sony Pictures often strikes one (or a few) new prints when they work on restorations.
Also, the 70mm print(s) for a 1977 L.A. and New York re-issue (not sure if other cities ever played it) were advertised as "Now...for the first time ever! Supersonic 70mm and Super-mind-blowing Ultrasonic Stereo." Anyone know what that was? Different from Quintaphonic?
