To answer my own question:

5 reels

Dolby Stereo SR

no attached/enclosed trailers

Technicolor depot

My print (#10) had leaders cut on R1-2-3, but leaders on R4 and R5 were uncut (though I had to cut them to run it on a platter). Weird.
