Of course they don't, but it's supposed to be subtle. If you ask, I'd bet they don't even notice 3 channels or 5 channels. Want to go back to mono if they don't notice? And judging from my experiences when things go wrong, people don't even notice when channels are completely missing.
But that's not the point. There's a subconscious effect that the overall presentation and channel distribution has on the audience. This is especially true as sound mixers have attempted to create a sound field, rather than having discrete sounds emanating from particular speakers, as they did in the 1950s-60s. And when it's done properly it makes a difference, although out of all the additions to cinema sound in the last 20 years, ES/EX is probably the most subtle.
Would I spend money on ES/EX if I wasn't upgrading anyway? Definitely not. But if I had another reason to upgrade the sound or if I were building a new screen, I would definitely put in that channel.
